Park West Care andRehabilitationCenteris seeking aMDSCoordinator, RN, to join our dedicated team at our center located in (CITY, ST). (CENTER DETAILS)
Responsibilities include:
* Complete nursing assigned MDS items, RAPS, PPS and Care Plans.
* Direct the facility interdisciplinary RAI process while following company guidelines for completion, computer encoding and electronic state transmission of the MDS.
* Participate in the resident care plan meetings.
Job Requirements : * Must be a Registered Nurse currently licensed by the State.
* Experience as a MDS Coordinator, completing MDS, RAI process and care plans with demonstrated knowledge of RUGS optimization.
* Knowledgeable of regulations and guidelines governing long-term care.
* 1 year experience in long term care or skilled nursing center.
* Strong communication, organizational and time management skills.
