Social Worker Job in Phoenix (Arizona, USA, United States)

Job Description:

Provides social work services for facilitating solutions to the medically related psychosocial/environmental problems of patients and families. Ensures the continuity of patient care by implementing interventions that include ongoing assessment, counseling, referral, information and guidance during hospitalization and outpatient treatment. Maintains positive working relationships with vendors and government agencies to access services for patients. The individual must demonstrate knowledge of the principles of growth and development of the life span. The ability to assess data reflective of the patient's status and appropriately interpret information relative to the patient's age-specific needs is required.

There are two positions available. The first position directly supports the pre surgical evaluation practice (POE). The second position initially supports an inpatient float assignment and will eventually support newly opened medical beds in an inpatient setting.

Basic Qualifications:

Master's degree in Social Work.

Other Qualifications:

Minimum two years of experience in a hospital or medical setting preferred.

License or Certification: Active license through a qualified State Board of Behavioral Health Sciences Examiners; Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) credential preferred. Must obtain license through the AZ Board of Behavioral Health Examiners within sixty days of hire and maintain thereafter. Basic life support (BLS) certification obtained within 30 days of hire and maintained thereafter
