Case Manager PRN Job in Jacksonville (Florida, USA, United States)

Job Description:

The Case Manager provides utilization review, discharge planning, and/or social work services to patients and families throughout the continuum of care. Utilization Review is guided by the Hospital’s Utilization Review Plan. The case manager will: 1. Work collaboratively with the interdisciplinary health care team to guide appropriate allocation of the hospital’s resources while providing high quality care to each patient in a cost-effective and timely manner across the continuum of care from admission through discharge. 2. Determine the admission status for all hospitalized patients and monitor the patient’s need for continued stay. 3. Monitor for inappropriate or untimely clinical services. Facilitate appropriate use of resources within the context of the current care delivery system and recommend changes which could optimize resource utilization. Discharge Planning is guided by the bio/psychosocial/environmental needs of patients and families. The case manager will: 1. Work collaboratively with the interdisciplinary health care team to develop an appropriate discharge plan that will meet the needs of the patient. 2. Follow appropriate steps of discharge planning which includes assessment, plan, intervention and evaluation of the discharge plan. Social Work Services is guided by the psychosocial/environmental needs of patients and families. The case manager will: 1. Utilize social work services that include: assessment, counseling, referral, education, and support during hospitalization and outpatient treatment. 2. Work collaboratively with the interdisciplinary health care team to develop an appropriate plan of care that will meet the psychosocial/environmental needs of the patient.

Basic Qualifications:

Must be either: 1.Registered Nurse, BSN is preferred 2. Social Worker, Master’s degree from a School of Social Work accredited by the Counsel on Social Work Education, LCSW is preferred. (Florida License as a Clinical Social Worker may be required for certain assignments.)

Experience: Minimum of one year clinical experience. Experience with utilization review, discharge planning, and/or adult medical outpatient social work preferred.

Schedule Details: 8-4:30 M-F
